Includes blank application form [at foot of sheet] to record information about payments and membership of An Cumann Cosanta and bears the handwritten signature of the Secretary of the Committee of Management Micheál Ó hAnnrachain [Michael O'Hanrahan, who was second in command of Dublin's 2nd Battalion under Thomas MacDonagh; he was executed by firing squad on 4 May 1916]. Containing the following information: "An Cumann Cosanta Society has been started in order to protect Irish Volunteers and others against the loss of their livehood on account of their connection with the Volunteer Organisation. Under the Scheme Volunteers and other persons connected with the different forms of Irish activity can insure themselves for various sums so that in case of dismissal caused by action properly taken in the forwarding of their propaganda they may be indemnified against their loss".
