Notes:The National Progressive Democrats was a new party founded on 16th May 1958 by Noel Browne and his fellow Independent T.D., Jack McQuillan. The party won two seats in the 1961 general election, but was dissolved in 1963.

The document is on 18 foolscap mimeographed pages, with yellow covers, stapled together. An application form on a smaller page is loosely inserted.
