Sir Joseph de Courcy Laffan (1786-1848) gained eminence as a medic through his aptitude in treating fever stricken British troops thus securing his appointment as physician to the forces, serving the Duke of Kent in the Peninsular Wars and gaining a baronetcy for treating George IV's brother, the Duke of York, for dropsy. This illness, now commonly referred to as edema, was treated for the first time with digitalis by William Withering. The clinical results were summarized in his great 1785 work on hte foxglove wherein he describes digitalis toxicity in detail. The lethal dose is a mere three times more than the optimum dose is and so, especially in the mid nineteenth century, treatment required true clinical experience. this was obviously suited to De Courcy Laffan who had gained his medical qualifications in the use and effectiveness of this drug. The treatise outlines a medical history of the use of digitalis with special reference to Withering's work on angina pectoris etc. and more importantly on 'Hydrope', known to the eighteenth century as 'dropsy of the chest' and in our age as pulmonary edema.
