The form was created by James Crean, Acting Inspector General, and forwarded to John O'Meara. Shields includes that his uniform belt, bandolier, bayonet, putties, and cap were not returned to him on his release.
Shields gives his address as "5 Lr Sheriff St. / Dublin". He signs as "Shields" but is addressed in the form, and recorded in the 1911 census, as "Shiels".
