In two parts, each with separate titlepage, pagination and register; the first part, 'The memorial of the Church of England', bears the imprint: "Printed in the flourishing year of the Church, 1711", and the second, 'An impartial account', has the undated imprint: "London printed; and, re-printed and sold by E. Lloyd, Dublin".
