The form was created by James Crean, as Acting Inspector General. Phillips notes that he was arrested by a Captain Clughan of the Royal Irish Regiment and detained from 30th April to May 12th. Phillips complains that his house was raided and that his personal belongings, such as his uniform, have not been returned.
